I am sorry but that would still go over the power limit with even generous umbrella rules. As a reminder, anything that a basic human cannot achieve without diligent training/study counts as a power.

Azathoth physically manifesting and speaking goes pretty far beyond the parameters of a normal Persona summon, so that would be a separate ability. Tyrant chaos is also a separate ability. Tentacles that can do physical attack stuff are an ability. Assorted resistances are an ability. And energy stream/healing powers are also a separate ability. This would total up to five abilities, and three would need to be chosen out of those.
